Could the head be colliding with other bits of the puppet? Maybe that makes the attachment fail. Is collision set to off? No idea if this is even a thing that can happen....

Scope in one more level so that you only have the head-segment of the puppet available to you. That’s where you sculpt your head. Sculpting the nose separately isn’t advisable.
