How do I stop my puppets head from falling off?

I can’t seem to stop my puppets head from falling off when I press play. Have grouped or ungrouped something wrong? Thank you.

  Take the object you want group with the head from the puppet with R2, then go to puppets head but hold R2 and press L1 and X to scope in. Then it is grouped.

  You might have deleted the joint between the neck and the head. You can re-join them and asign the new joint inside the Puppet Tweak Menu as follows:
  
  Scope into the Puppet, Shift+Square over the blue puppet base. Go to the fourth tab (Body Structure), press the the bone icon on the top and then the Head.
  
  This should tell the Puppet engine where its head is! :)
